To your right is a copy of form IT-QEE-TP1, page one. Click on this image and provide the requested information for the first two pages. Print the form, then follow the mailing  instructions on page 3. When the form is returned to you, mail it along with your donation to the address given on page 3 which is the PO Box listed at the bottom of this page.

A blank TP1 form and/or the TP1 instruction pages can be printed out from our forms page.

SEE your tax preparer to determine the eligibility of BOTH a FEDERAL AND STATE tax deduction.    

Georgia State Income Tax Credit up to:
$1,000.00 for an individual
$2,500.00 for a couple
75% of tax due for a corporation    

REMEMBER......We issue you a TAX CREDIT not a form for a tax deduction. Your SSO1 is the SAME AS CASH money.
